The pandemic year did two things to the regenerative-medicine field. It shut down patient-facing clinical operations that depended on tight, in-person logistics. And it pushed laboratories and clinicians alike to look harder at preparations that did not require living cells in transit. The first effect was disruptive in obvious ways. The second was more subtle and, looking back from the perspective of 2026, more lasting.
Cell-based therapy programmes that relied on shipping autologous or allogeneic cells between clinic, processing facility, and clinic again ran into specific operational problems during 2020. Border closures created delays that mattered for cells in temperature-controlled transit. Freight constraints created scheduling problems where they had not existed before. Hospital and clinic capacity was redirected toward urgent COVID-19 care, which in many regions meant elective or research-stage procedures were paused, sometimes for months at a time. Cell-therapy programmes have always carried logistical complexity. The pandemic exposed how much complexity.
Programmes built around stable, characterised, non-cellular biological preparations did not run into the same set of problems. A characterised cell-free preparation can travel under standard biological-material shipping conditions, with longer windows of stability and lower temperature-sensitivity than living cells. It can be stockpiled to some extent. It is less dependent on tight scheduling between clinic and processing facility. None of this is to say cell-free preparation programmes were unaffected by the pandemic. They were, in many cases significantly. But the constraints they ran into were not the same constraints.
The conversation across regenerative medicine through 2020 reflected these realities. The number of registered clinical trials involving extracellular vesicles, exosomes, and other non-cellular biological preparations grew significantly during the pandemic period. Some of that growth was driven by COVID-19-specific research, in which secretome-derived preparations were being studied as candidates for immune-modulation in severe disease. Some of it was unrelated to COVID and reflected work that had been in progress for years and was reaching trial stage. Either way, the publicly visible pipeline of cell-free preparation research expanded substantially.
Investor and operator interest in the cell-free end of the regenerative-medicine space grew in parallel. Several companies in the cell-therapy space publicly broadened or pivoted their development pipelines to include secretome-derived candidates. Existing cell-free preparation companies accelerated their fundraising and trial-registration activity. The reasons for this varied by company. The collective effect was a noticeable shift in where the regenerative-medicine field was directing capital and attention.
Wellbeing’s own research programme through 2020 was less disrupted by the operational dimension of the pandemic than many cell-based programmes were. The work was already based on stable, characterised, non-cellular preparations. The cell-source side of the programme, as discussed in earlier posts in this series, continued to evolve through this period. By the end of 2020 Wellbeing’s working position was closer to where the programme sits today than to where it had been at the start of the year.
Worth noting carefully: Wellbeing does not claim that cell-free preparations are categorically superior to cell-based ones. The two approaches are not in competition for the same applications. Cell-based therapies remain essential in fields such as oncology, where the biological work is structured around the cell itself. Cell-free preparations are better suited to applications where the goal is to support the body’s own repair and signalling processes. The pandemic did not make either approach right. It made the operational profile of each more visible.
For readers tracking the longer arc of how regenerative-medicine research moved through the 2020s, 2020 is the year the field’s centre of gravity shifted in a direction that had been forming for the previous decade. The pandemic accelerated something that was already happening.
